If you are a Schwab client, you can click the Headset icon in the upper right of the page. Then click Forms & Applications. Then click the blue PDF link Charitable Gift Donation. You can fill this out, sign & date, and then upload by clicking the envelope icon in the upper, right of the page. The Schwab DTC information is clearing number 0164, code 40.
